Note: If you are applying to add a chapter account, please provide the university name as the First Name and either
KKY or TBS (depending) as the Last Name. Abbreviations of university names are preferred (e.g., U of Missouri, Kansas State, Northwest Missouri State). In general, leave off "University" or "College" if it is the last word of the name, and abbreviate it with "U" if it is the first word.
